What to Expect During Your First Visit
You have booked your appointment, now what?
Our goal is to provide a comfortable, welcoming, and attentive visit.
How to prepared for your visit
To maximize your time with the doctor, your initial office paperwork is emailed to you before your appointment. This allows you to fill out your paperwork at your own pace and in the comfort of your home. If you have not received the emailed paperwork, you will be asked to complete the initial office paperwork when you first visit our office.

Initial office paperwork can also be completed during your first office visit.
The information you provide will assist us in assessing factors that may be contributing to your condition, as well as ensure recommended therapies are consistent with your current General Health Care program.
ADDITIONAL PATIENT FORMS (Not first time patient forms)
The Consultation
- Dr. Erickson will complete a consultation, a basic physical and neurological examination (based on your condition). If additional diagnostic tests are needed, you will be advised and referred during your consultation.
An initial treatment may be provided during this visit.
Health Plan Strategy
Once your evaluation is complete, the doctor will discuss recommended treatment plans, as well as any questions and concerns that you may have.
